Required before your child uses Sparky.

1. About This Form

This Parental Consent and Authorization Form is provided in compliance with the Children's Online Privacy Protection Act (COPPA), 15 U.S.C. § 6501 et seq., and the FTC's COPPA Rule, 16 C.F.R. Part 312.

By completing this form, you, as the parent or legal guardian, authorize Unimatch LLC to collect and process limited personal information from your child as described in the Sparky Privacy Policy, which you confirm you have read and understood.

2. Parent or Guardian Information

• Full legal name of parent or guardian
• Email address (for verification)
• Relationship to child: Parent / Legal Guardian / Other
• Confirmation that you are 18 years of age or older

3. Child Information

• Child's first name or nickname (not full legal name)
• Child's age range: 6–7 years / 8–10 years
• Preferred language

We do not require or collect your child's full legal name, school, address, photograph, or any information beyond what is listed above.

4. What You Are Authorizing

By submitting this form, you authorize Unimatch LLC to:

• Collect and process your child's voice input during Sparky sessions for the sole purpose of generating AI responses
• Retain text transcripts of your child's conversations for a maximum of 30 days for safety review purposes
• Allow a limited number of Sparky safety team members to review conversation transcripts where a safety concern is flagged, under strict access controls
• Share de-identified transcripts with our AI service provider solely to generate responses (the AI provider is contractually prohibited from using this data for model training)

5. What You Are Not Authorizing

• Use of your child's data for advertising, behavioral profiling, or commercial purposes
• Sale or sharing of your child's data with any third party for purposes other than operating Sparky
• Retention of your child's audio recordings beyond the immediate transcription window
• Collection of any data beyond what is described in Section 4 above

6. Your Rights

At any time, you may:

• Request to review all personal information collected about your child — email privacy@sparky.ai
• Request permanent deletion of your child's data — we will fulfill this request within 30 days
• Withdraw this consent at any time by emailing privacy@sparky.ai— withdrawal will terminate your child's access to Sparky
• Refuse to permit further collection of your child's data while retaining your own account

7. Verification

To verify your identity as a parent or guardian, you will receive a confirmation email at the address provided. Your child's access to Sparky will only be activated after you click the confirmation link in that email.

Sparky uses email-based verification as its parental consent mechanism for the beta program. By confirming your email, you certify that you are the parent or legal guardian of the child and that the information provided is accurate.

8. How to Submit

Please email this completed form to legal@sparky.ai with subject line: Beta Consent Form — [Child's Nickname].

Include in your email: your full name, email address, relationship to child, child's first name or nickname, age range, and preferred language. Confirm that you have read and agree to the Privacy Policy and Terms and Conditions.
